About the Innovation Awards
At this year's Symposium, we will be presenting the following Innovation Awards:

Boehringer Ingeleheim Innovation Award
The GIM Symposium Planning Committee wishes to recognize an individual(s) who have enabled interprofessional collaboration, partnership and excellence in the provision of complex care and exhibit the University Health Network's values of Caring, Excellence, Teamwork, Integrity and Leadership.
Award will be granted to foster ongoing excellence, innovation or future research, to cover publications fees or the cost of membership at relevant conferences to contribute to the sustainability of this initiative.
Eligibility:
Criteria:
To be considered as a recipient for this award, the individual(s) or team has demonstrated two or three of the following:
- Excellence within their discipline in the coordination and continuity of care by collaborating with patients, families and other members of the healthcare team
- Initiative to take action within a team to provide care that addresses patients’ concerns and improves their quality of healthcare
- Leadership throughout the organization to promote innovation and excellence in clinical practice, education and research
- Application of new trends in healthcare, care models or technology that provide creative approaches in patient care services
- Promotes/inspires others to lead to foster a supportive and learning environment

Chandaria Fellowship Award
The Chandaria Fellowship Award will be presented to an individual who has enabled interprofessional collaboration, partnership and excellence in the provision of complex care and exhibit University Health Network’s values of Caring, Excellence, Teamwork, Integrity and Leadership.

Criteria:
To be considered as a recipient for this award, the individual has demonstrated two or three of the following:
- Application of innovative leadership that improves the quality of patient care and adds value to the Canadian Health System
- Evaluation in the clinical environment which resulted in a positive impact on healthcare outcomes, clinical practices, treatments, programs or services
- Outstanding academic achievement that has made a significant contribution to research, science, technology, health policy or integrated healthcare
- Commitment to a study or project in the Centre for Innovation in Complex Care (CICC) to improve the system and processes of care for patients with multiple problems to improve patient’s quality of life through innovative solutions that impact global healthcare

HOOPP Award for Leading Innovation in Front Line Care
In 2010 HOOPP was awarded one of the 10 Most Admired Corporate Culture Awards via a program that recognizes Canadian organizations for having a culture that has helped them enhance their performance and sustain a competitive advantage.
HOOPP believes that great people create great cultures, and great cultures create an advantage that helps generate outstanding results.
We are accepting nominations for the “HOOPP Award for Leading Innovation in Front Line Care”.
This award will recognize the vision and leadership of a team of front line healthcare workers who have broken down barriers to create innovation that led to improvements in patient care.
The award will consist of a team dinner (5 people) hosted by HOOPP at a local Toronto restaurant where HOOPP and the winning team can talk about the factors that fostered their innovation.
HOOPP will also feature the winning team and award on its website and newsletter that goes out to 250,000 HOOPP members. HOOPP will also provide award plaques to the team members and send a letter of recognition to your hospital board and management team recognizing your achievements.
Please consider carefully how your nominated team exemplifies innovation in patient care leading to improved leadership, empowerment, interprofessional team development, and improved quality care.
